Creating a Google Doc

This is how to create your own:
1. Go to https://docs.google.com/forms/u/0/
2. Click “blank”
3. Type in your questions and indicate which response you would like (ex. multiple choice, short answer, etc) for each question. Mark the “required response” button for each question.
4. After you create the questionnaire, click on “settings” inside the form, and choose “collect email addresses” --> this should ensure you are notified via email when a customer sends you her responses.
5. Click preview. Copy the link and save it to share with your customers :)
